We want to provide opportunities to help individuals achieve their goals and realise their potential, but we can only do this with your support.

The Exercise on Referral Project is for patients within the Shrewsbury Primary Care Network to access free structured programmes of physical activity or exercise under the guidance of Shrewsbury Town Foundation staff.  

The objective of this programme is to provide a positive introduction to being active which may act as the start to long-term behaviour change within patients. 

The Exercise on Referral project will begin with a personal, 1 to 1 assessment between the patient and STITC staff who will work together to design a specific exercise programme to meet the abilities and the health needs of the patient. 

About the programme

The Exercise programme will include access to Gym Facilities at Shrewsbury Town Foundation as well as the opportunity to participate in extracurricular sessions with the Foundation such as Walking Football, Walk & Talk, Extra Time & Kick Cancer.  

Patients will be invited to participate over a number of weeks, free of charge before given an end of programme assessment to check your progress before re-evaluating how they would like to continue exercising. 

Patients will need to be a registered patient at one of the 16 surgeries within the Shrewsbury Primary Care Network.  

The Exercise Referral Project will take place at Shrewsbury Town FC, Montgomery Waters Meadow, Oteley Road, Shrewsbury, SY2 6ST.  

Where possible, this programme will be delivered in a face-to-face environment, however, where national and local covid 19 restrictions may dictate, both parties will work together to deliver an alternative programme, for example, using online platforms. 

By taking part in the Exercise on Referral project, patients will see improvements in their: 

  • Knowledge, confidence and self esteem 
  • Quality of life 
  • Levels of physical activity 
  • Sustainable improvement in patients’ lifestyles and behaviours 

The long-term benefits that patients could see from the programme are to increase the participants life expectancy and quality of life, reduce health complications and ultimately reduce their use of health services within the area.

How old do I need to be to take part in the EOR Project? 

There is no upper or lower age limit attached to this project, where possible we will aim to make it accessible to all ages and abilities. 

Can I take part in this project if I don’t have a diagnosed health condition? 

Yes – you do not need a pre-diagnosed health condition in order to access the support of the EOR project. The EOR aims to improve the health of those who may also be at risk of developing a health condition. 

Can I just turn up to a session? 

All patients are required to attend a 1 to 1 interview with one of our health & wellbeing coaches in order to provide a detailed and tailored plan to meet the needs of the patient. This can be done by contacting [email protected] or via 01743 289177 ext 216.
