The Exercise on Referral Project is run by Shrewsbury Town FC Foundation in partnership with the Shrewsbury Primary Care Network allowing patients to access a 12-week, free structured exercise programme.

The programme begins with a personal, one to one, welcome chat between the client and a member of STFC Foundation’s health and wellbeing team. Our coaches will work with clients to design specific exercise programmes that meet their health needs, requirements and goals.

About the programme

The 12-week funded Exercise on Referral programme will include access to our Open Gym and classes at Shrewsbury Town FC Foundation, as well as the opportunity to participate in a variety of other Health and Wellbeing sessions, such as, Extra Time Hub, Walk & Talk, and Walking Football.

During the 12-week funded programme, clients will have continued support from the team and will undergo a Fitness MOT at the start and on completion of the programme, monitoring their progress before reviewing how their health and wellbeing journey will continue with the Foundation.

Clients will need to be a registered patient at one of the 15 surgeries within the Shrewsbury Primary Care Network in order to participate on the FREE Exercise Referral scheme.

The Exercise Referral Project will take place at Shrewsbury Town FC, The Croud Meadow, Oteley Road, Shrewsbury, SY2 6ST.

By making a commitment to the Exercise Referral project, clients will see improvements in their:

  • Knowledge, confidence and self esteem
  • Quality of life
  • Levels of physical activity

If clients decide to continue with the Foundation after completion of the 12-weeks, the long-term benefits that patients could potentially see are: reduced reliance on medication, sustainable improvement in lifestyle and behavior, reduced health complications and ultimately decreased pressure upon health services within the area.

About the Shrewsbury Primary Care Network

The Shrewsbury Primary Care Network is made up of 11 GP practices supporting over 124,000 patients in and around Shrewsbury, Shropshire. By working together practices share decisions, governance, workforce, money and other resources meaning we can proactively offer more effective and efficient services that meet our patient’s needs.

We employ a range of additional health care professions including physiotherapists, paramedics, dietitians, social prescribers and health and wellbeing coaches to name just a few who work across multiple practices in and around Shrewsbury. This diverse workforce means we can support patients to access the most appropriate health care professional in a timely way.

FAQ: 

How old do I need to be to take part in the Exercise Referral programme?

There is no upper or lower age limit attached to this scheme, where possible we will aim to make it accessible to all ages and abilities.

Can I take part if I don’t have a diagnosed health condition?

Yes. You do not need a pre-diagnosed health condition in order to access the support of the Exercise Referral programme, however, if self-referring, a doctor’s note is required prior to exercise. Exercise Referral aims to improve the health of those who may also be at risk of developing a health condition.

Can I just turn up to the gym?

Clients are welcome to visit the Open Gym anytime, however, please be mindful that the Health and Wellbeing Coaches do have appointments and bookings throughout the day.
